its an old discussion but weare having the same Problem with Epson and Fujitsu dot matrix printers on 2008R2 Printserver and 2008R2 Citrix XenDesktop clients on VMWare virtual machines.
After adding the printer to the user session the printouts work fine for a few hours, days or weeks. Randomly the paper size is set to any interated paper size like letter, a4,a2. With an extra tool we figured out, that the paper size ID from the custom and driver generated forms changes a few times a day for about one or two minutes. IDs from itegrated forms dont change. Only deleting and re-adding the printer will fix it temporarily.
We are having the same problem on 2003 Server / Clients on Citrix Xenapp before. Increasing the IRPStackSize registry paramter didnt solveit but the frequency of the issue was reduced.
